Person Centered Planning

Every person receiving ACL-BB services must have a person centered plan completed.

Person centered planning should take place with the client as well as the people he or she wishes to invite to the planning meeting. This could include family members, friends and support staff, including Day, Residential staff and Community Service Workers.

Personal preferences should be considered when preparing for this formal planning process. If a formal meeting is not the planning tool of choice for the person other creative methods should be developed and utilized.

Employees are expected to assist the person with preparation for the meeting (or planning process) in advance by discussing his or her dreams, interests, wishes and personal goals.

The plan must be documented with specific goals for accomplishment by the person. Necessary supports should also be identified.

The plan must be reviewed regularly. The review time frame may be set out in the plan. At minimum it is suggested that an annual review take place.

Person Centered Planning should address the client’s financial, residential, day program, social and medical needs.

Employees are expected to assist the person with the accomplishment of the stated goals as part of their job duties.
